Creating a template
- Click the Configuration menu icon (
) on the left vertical pane.
- Click Template Management.
- Select a site from the Selected Site drop-down list. The organization name is displayed in bold and its associated sites are displayed below.
- Click Create New Template button.
The Add Template page is displayed. It shows two tabs: Edit/Create and Preview. The Edit/Create tab is opened by default. This tab enables you to create or edit a template. The Preview tab enables you to preview the template. The Preview tab displays the preview only after you have added controls on the template.
The Edit/Create tab shows the steps sequence at the top:-
The 0 Lines indicates the section where you can add number of lines to the template.

The 1 Layout indicates section where you can create a layout for the template by adding number of columns.
Note: Maximum of three columns can be added in a row. -
The 2 Controls indicates section where you can add fields to the template.

The 3 Save indicates section where you can view the template and save it.
The Command section enables you to add a new line, i.e., rows to the template by clicking the plus
sign next to the Add a new line label.
The Visual section displays the preview of the template. For example, as you add a new line, it is shown below the already present line.

- Click the plus
icon next to the Add new line label to add a new row in the template.
The new row is added below the existing line.Note:Click the delete
icon to delete a line in the template.
You can move the lines up and down by using the arrow icon. Click the down arrow
icon to move a line down. Click the up arrow
icon to move a line up.
- Click Next.The Layout section is displayed. This section allows you to add columns to a line. By default, the first line is selected.Note:
The green check box
indicates that the line below is active and can be edited to add or reduce columns.
The orange check box
indicates that the line below cannot be edited. Click the orange check box to edit a line.
- To add a column to a line, from the Number of columns field, click the plus
icon. Add the required number of columns.
Note:You can add up to three columns in one line. To add columns to the other line, please select the orange check box
for the particular line and then click the plus
icon. To reduce the number of columns, click the minus
icon.
- Click Next.
The Controls section is displayed. This section allows you to add controls to the columns.
- Select the orange check box
for the required line that you want to add controls to and then click the column in which you want to add the controls.
The Select a control window is displayed.
- In the Select a control in the list below field, select the control from the drop down list and enter the property information for the control as indicated in the table below:

Select this control to add header to the template.
Header Text
Enter the text for the header.
Description
Enter a description for the control.
Subtitle Select this control to add subtitle to the template.
Subtitle Text
Enter the text for the subtitle.
Paragraph Select this control to add paragraph to the template.
Paragraph Text
Enter the text for the paragraph.
Note:A text editor is provided with various formatting features such as bold, italic, heading styles, alignment, numbered and bullet list that you can use to format text. You can add video to your text. You can also add hyperlink. You can change the view of text editor to HTML editor if required.
Images cannot be added to the template.
Text input Select this control to text input field to the template.
Label text
Enter the text for the label of the field.
Placeholder
Enter the text for the placeholder of the field.
Default value
Enter the default text to be shown in the text field.
For example, when you have First Name field, then you can add text such "Enter your first name".
Email
Select this control to email to the template.
Label text
Enter the text for the label of the field.
Placeholder
Enter the text for the placeholder of the field.
Required
Enable the Required check box if you want to mark the control as mandatory field.
Description
Enter a description for the control.
Date
Select this control to add date field to the template.
Date format
Select the date format in which you want date to be displayed.
Note:The date formats available for selection are: dd-MMMM-yy, yyyy/MM/dd, dd.MM.yy, and short date.
Label text
Enter the text for the label of the field.
Required
Enable the Required check box if you want to mark the control as mandatory field.
Description
Enter a description for the control.
Text area Select this control to add text area field to the template.
Label text
Enter the text for the label of the field.
Required
Enable the Required check box if you want to mark the control as mandatory field.
Description
Enter a description for the control.
Radio
Select this control to add radio button field to the template.
Add new radio
Enter the name of the radio button option and then click Add.
Click the delete
button to delete the option.
Click the down arrow
button to move radio button option down.
Click the up arrow
button to move radio button option up.
Label text
Enter the text for the label of the field.
Required
Enable the Required check box if you want to mark the control as mandatory field.
Description
Enter a description for the control.
Check box Select this control to add check box field to the template.
Label text
Enter the text for the label of the field.
Required
Enable the Required check box if you want to mark the control as mandatory field.
Description
Enter a description for the control.
Basic select
Select this control to add drop-down field to the template.
Add new options
Enter the name of the Basic select option and then click Add.
Click the delete
button to delete the option.
Click the down arrow
button to move radio button option down.
Click the up arrow
button to move radio button option up.
Label text
Enter the text for the label of the field.
Required
Enable the Required check box if you want to mark the control as mandatory field.
Description
Enter a description for the control.
No control Select this control to leave the column blank. This option is selected by default.
As you select the control, its preview is displayed in the Preview section.
- Click OK
The Select a control window is closed and the control is added to the column in the template and it is displayed on the Controls page.
Tip:You can click Preview to view the display of the template.
Repeat the previous steps to add other controls.
- When you finished selecting all controls, click Next.
The Save section is displayed. It shows the preview of the form.
- In the Name field, enter the name for this form. The template name contain maximum up to 50 characters.Note:
The name of the form must be unique. Duplication of form name within the same site is also not allowed.
Tip:It is recommended to give the form meaningful name that allow the users understand the purpose of the form without reviewing the description of the form.
- In the Description field (mandatory), enter the description for this form. The template description can contain up to 255 characters.
- Select the Template Type (mandatory).
- Select Request to create a form based template that can be used to provide the users with instructions, request users to provide information, and collect required information back from the users to enter a request for a dedicated use case.
- Select Report to create a form based template that can be used to create clinical report.
- Optionally, select the Make this template visible to other sites in the Organization check box to make this template visible to other sites in the organization to which you belong.
- Click SAVE. Note:
The SAVE button is enabled once you enter details in the Name and Description fields.
The template is saved and the following confirmation message is displayed: "Template created successfully".
The newly added template is displayed on the Template Management tab.
If the template is not created successfully, the following notification is displayed: "An error occurred while creating the template. Please try again."
If the site admin creates a template with an existing template name within the same site, the following notification is displayed: "Template name already exists."
